Client Case Manager I – Transitional Living Center

Job Responsibilities:

  1. Consult with clients one-on-one on a weekly basis on their progress and make detailed documentation for the file immediately after closure of the session.
  2. Maintain accurate records on each client on a reduced caseload: the size to be determined by the Manager of Client Services. This will consist of preparing a Master Action Plan on each client on his/her caseload after consulting with clients and counselors. Master Action Plans must have verifiable long-term objectives and strategies for their attainment. This will also consist of preparing a Monthly Action Plan, which will identify short-term objectives and strategies that will compliment the Master Action Plan.
  3. Participate in regular staffing meetings with counselors as assigned.
  4. Refer clients with specific needs to the appropriate counselor.
  5. Attend regular Case Management meetings and other meetings.
  6. Enforce rules, policies and procedures as appropriate.
  7. Screen clients for substance abuse, both random and for cause.
  8. Make a record of weekly and bi-weekly room inspections.
  9. Random file reviews.
  10. Make log entries for continuity of support services.
  11. Review and sign HUD forms, client probation evaluation forms and client weekend passes.
  12. Participate in trainings as assigned.
  13. Assume the responsibility of all clients by promptly responding to any unsafe situations.
Other Job Responsibilities:
  1. Other duties as assigned.

Qualifications:

B.A. or B.S. in Social Science. 0-1 years working experience in the field of Social Science as a Case Manager. Good computer skills and knowledge of Microsoft Windows, Microsoft Word and Excel.
